Required Documentation

Upon acceptance into our school, the following forms must be submitted to TMSA. Additionally, all immunizations must meet the requirements of the State of Massachusetts.

• Child's birth or baptismal certificate
• Previous school records with form from teacher
• Health records (required by state law)
